Kalispell TRT is a hormone therapy and peptide clinic in Kalispell, serving patients by telehealth across Montana. They wanted their own team answering the phone — not an AI standing in front of it. That's what got built.

What they needed

A clinic's front desk isn't a call center. The people who answer are the same people checking patients in, and the calls that get missed are the ones that arrive while they're doing that — plus every call that comes in after close.

The brief was specific, and it ruled out the obvious approach: staff answer first, always. An AI that picked up before a human did would have been the wrong build, however impressive the demo.

What we built

Person-first routing

Conditional forwarding rings the staff phone for about twenty seconds. The AI picks up only when nobody can. Verified end to end with the clinic on the line during staffed hours.

Booking that runs itself

Consults land on the real calendar with a confirmation text, 24-hour and 1-hour reminders, and a public booking widget that blocks overlapping slots so a double-booking isn't possible.

A hard clinical boundary

The assistant handles front-desk work only — hours, location, what the clinic treats, booking, messages. Anything clinical is declined and routed to staff by design, and that scope is written into the contract, not just the prompt.

Calls, texts, and chat in one place

The same front desk answers the phone line, SMS, and the website chat, so a patient who starts in one channel isn't asked to repeat themselves in another.

Three things that actually happened

The AI declined a clinical question and handed it to staff

During the first live week, callers asked the assistant clinical questions. It refused to answer them from its own knowledge and routed the callers to staff — exactly the designed behavior. The questions showed up in the knowledge-base log as unanswered, and they were left that way on purpose, with the clinic's agreement.

That's the part worth reading twice: the gap was left open deliberately. An AI front desk that quietly starts answering medical questions is a liability, not a feature.

Three requested changes went live the same afternoon

The clinic emailed three refinements at 2:00 PM — how the assistant pronounced the website, leading pricing answers with the free consult, and how it handled a lab-work question. All three were deployed, test-called, verified, and confirmed back to the clinic that same afternoon.

No ticket queue. The person who built it is the person who changes it.

The owner booked a real patient himself, untrained, and got it right first try

Within the first week, Jason booked a patient consultation directly in the calendar without being walked through it, then messaged to ask whether he'd done it correctly. He had — right calendar, appointment confirmed, telehealth link attached. The confirmation text reached the patient within seconds, the reminders armed themselves, and the public widget blocked the overlapping slot.

A system the owner can drive on instinct is worth more than one that needs its builder in the room.
